Sports packets and WIAA physical forms can be picked up in the main office or WIAA Physical Form. When the packets are completed, students turn them in to Mrs. Hamre in the student store.
In order to participate in the athletics program, students are required to purchase an ASB card for $25. This is a one-time fee. These cards can be purchased online through your Family Access account or in the main office before school or during lunches. Students will need to have a new Athletic Packet filled out each school year. Physicals are good for two (2) years but students should check with Mrs. Hamre to see if their proof of a physical is on file, and/or if they need a new one. It is recommended that parents keep a copy of their student’s physical at home as well as turning in a copy to the office.Legislation has been passed requiring parents to sign a Concussion Information Form. This form must be completed in order to participate.
