Daily health assessment forms / temperature screenings
A daily health assessment (also called “attestation”) form is required for all students who participated in face-to-face learning.

Parents are encouraged to complete the health assessment document form prior to sending their child to school.

Every student must turn in a completed health assessment form or complete a health assessment given by a staff person prior to entering the school building.

The District will provide printed copies of the health assessment form in English and Spanish.

If a student has a fever (>100.4 degrees F) upon entry to the building, they will be taken to a “temporary observation” area.
